Dan, I'll offer a quick answer, Grant can elaborate:

The tenon is stepped, that is, the top of the tenon is cut off, at the exact thickness of the extension block. The extension block thus 'rides' on top of the tenon. You can glue it on, and then drill two holes at an angle down through the fingerboard extension and into the tenon, and glue-in two wooden dowels, thus pinning it all together solidly.

You may want to make the fingerboard extension a tiny bit thicker than necessary, then, after the glue dries, sand the fingerboard extension down flush with the top of the neck. That will ensure a totally flat fingerboard extension - in the same plane as the top of the neck - for the fingerboard to sit on.
